Part 1 - Completed by Parent/Guardian

3. Family-Breakthrough Partnership Agreements

4. Most recent report card

5. Email addresses for students recommending teachers/counselors

If applying online, after completing Part 1 you will receive an email with instructions on how to handle the next step, submitting the student supplement.

 

Part 2 - Completed by Student

Student supplement:

1. Essay

2. Short Answer

3. “Express Yourself” Video

🅑 INTERVIEW

If selected to advance, a Breakthrough team member will contact you and your parents/guardians to schedule the interview session.

Family participation:

• Students are interviewed

• Parents/Guardians participate in a Q&A session on expectations
